5. Cancellation and No-Show Policy
We understand that circumstances sometimes change. However, appointment times are reserved specifically for each patient.
Origin Health Physical Therapy PLLC requires at least 24 hours’ notice to cancel or reschedule an appointment.
If you cancel with less than 24 hours’ notice or fail to attend your appointment without notice, a $75 no-show or late cancellation fee may be charged.
This fee helps compensate the clinic for lost appointment time that could have been offered to another patient.
Cancellation or no-show fees may be charged to the payment method on file or requested prior to booking future appointments.
6. Fees and Payment
Payment for services is due at the time services are provided unless otherwise agreed in advance.

If an invoice or payment remains unpaid, the clinic reserves the right to delay future appointments until payment is settled.
